Refund for Dropping a Program (rev. 2012)

Requirement(s): Duly accomplished dropping form signed by the Dean and the Registrar; Official Receipt; Certificate of Registration

Location: Cashier’s Office of the Campus

Fee: None

Steps:

  1. Present requirements to the Cashier’s Office. The cashier will validate the presented requirements based on the existing policies on refund.
  2. After validation, you will be required to return on the date of release. The preparation of refund usually takes 3 working days.
  3. On the scheduled date, go to the Cashier’s Office and receive the refund. In case you are sending a representative, authorization letter and valid ID of representative will be required.

Issuance of Library Card (rev. 2012)

Requirement(s): Certificate of Registration and two pieces 1x1 photos (not computer generated, with white background

Location: Campus Library

Fee: None

Steps:

  1. Present requirements to the Library personnel. Blank student information sheet and library card will be issued to you.
  2. Fill up student information sheet and library card, then submit to the library personnel.
  3. Receive the Library Card.
  4. Sign the log book.

Issuance of Medical for OJT

Requirement(s):School ID or Certificate of Registration

Location: Medical/Dental Clinic

Fee: None

Steps:

  1. Present requirements to the Campus Nurse.
  2. Fill out medical examination form.
  3. Submit the filled out form. Upon submission, the nurse will give your existing medical records form filed in the clinic. Present your medical records form to the attending physician before undergoing medical assessment.
  4. Undergo medical assessment. The attending physician will record the findings on your medical records form.
  5. Present the updated medical records form to the nurse. The nurse will prepare the medical certificate based on the findings of the physician.
  6. Sign on the clinic’s log book and receive the medical certificate.

Issuance of Transcript of Record

Requirements: Duly signed clearance form

Location: Registrar’s Office of the Campus

Fee: P50.00 per page

Steps:

  1. Secure request form at the Registrar’s Office.
  2. Fill out and submit the request form to the Registrar’s personnel.
  3. Pay the TOR Fee at the Cashier’s Office located at the Administration Building.
  4. Present the official receipt (OR) and the request form to the Registrar’s personnel. The Registrar’s personnel will indicate on the OR the date of release of the TOR. The release of TOR usually takes three working days during off-peak seasons in the Main Campus and five (5) working days in satellite campuses.
  5. Return on the scheduled date of release. Bring the duplicate copy of the request form the OR.

Application for BPSU-CAT

Requirements:(1)Certified photocopy of Form 138 (Report Card), transcript of records (for those who have already taken academic program in other college or university) or certification of grades from the school principal; (2) Two (2) pieces passport size photos; (3) Photocopy of Certificate of Good Moral Character

Location: Guidance Office, BPSU Main Campus

Fee: P250.00

Steps:

  1. Present requirements to the Guidance and Admission personnel at the Main Campus. The GAO personnel will verify the requirements presented.
  2. After verification pay the Processing Fee at the Cashier’s Office.
  3. Present the official receipt to the GAO personnel. A blank BPSU-CAT application form will be given.
  4. Fill out the application form and submit to the GAO personnel.
  5. Receive the test permit with the place, date and time of examination indicated therein.
